In 2010, an employee of Dr. Carl B. Holm complained to the Attorney General’s Medicaid Fraud Control Unit. Holm plead guilty to one count of provider fraud for billing Medicaid 67 times for surgical procedures when a tooth was simply removed non-surgically, according to Idaho’s KMVT News.

Dr. Carl B. Holm was sentenced to three years on probation and a $1,000 fine. He must also pay over $7,000 in restitution to Medicaid and nearly $3,000 to the Attorney General’s Office.

As part of the plea agreement, other counts against Holm were dismissed, including allegations of billing for fillings that weren’t provided or necessary or billing for X-rays that were of poor quality or of no diagnostic value.
