ADHA solicits nominees for student advisor award

Eligibility requirements and application procedures are provided.

To be eligible for nomination, the candidate must be:

• A current member of ADHA

& bull; The Advisor to the ADHA Student Member Organization of an accredited
dental hygiene program.

The candidate's application must consist of the two segments below:

• Application Cover Page

• Completed Application

Describing how the Student Advisor has promoted the profession of dental hygiene to the students she/he advises and how the Student Advisor has encouraged student and postgraduate membership in ADHA.

The application should not refer to the school or Student Advisor by name.

Dental hygiene students are encouraged to meet as a group and discuss the application questions. One or two persons could then be selected to compile the comments and complete one application for the nominee. The application should be typed and not handwritten.

Applications should be forwarded to the ADHA Member Services Division, 444 North
Michigan Avenue, Suite 3400, Chicago, Illinois 60611 and must be postmarked on or before February 15, 2006, to be considered. Only one candidate may be nominated from each program. The signature page validates the application and indicates approval of your dental hygiene student body.

The Council on Member Services will individually evaluate each application without group consultation.

The recipient of the ADHA Student Advisor Award and nominator will be notified by letter in April. This individual will receive a cash award along with a plaque provided through the generosity of Waterpik Technologies. The Student Advisor Awardee will be recognized at the 2006 Annual Session in Orlando, Florida.
