New online forms feature lets doctors create any form that can be completed anywhere using any device with data written automatically to patients' electronic health records.

Curve Dental, developer of Web-based dental software, has added online forms to its practice management software. Curve Dental’s online form capabilities enable doctors to create any form that can be accessed by any device in any location with all the data written automatically to the patient’s record.

“Our online forms are the easiest way to collect, process, and manage patient information,” said Ian Zipursky, president of Curve Dental. “Our customers are now able to quickly create any necessary form to collect the information they need and then have that data automatically written to the patient’s digital record. The level of automation we have added to a previously tedious task will significantly reduce the time a practice spends entering and updating patient information. In short, we have made the chore of onboarding new patients a chore no more.”

Leveraging the company’s 11 years of cloud-based development experience, Curve Dental’s online forms use drag-and-drop functionality to simplify the task and minimize the amount of time required to create a form. Any piece of information can be gathered by a form, such as contact information, allergies, medical history, and even a digital signature. After the form is created, the practice can send a secure link to a patient via e-mail. The patient accesses the form by clicking on the link and logging into a secure patient portal. The patient can complete the form using any digital device, such as a smartphone, tablet, laptop, or desktop computer. The form can be completed anywhere an Internet connection is available.

All of the data captured by the form is automatically written to the patient’s electronic health record in Curve Dental’s practice management system. Information gathered from the form replaces any existing information in the patient’s record. If the patient has moved and entered a new address in a form, the new address information will replace the existing information and be flagged for the practice to review. Patients who fail to complete a form within a specified period of time can receive reminders to complete the form via text or e-mail.

Curve Dental will be releasing this new functionality in stages; most features are available now, while others will be available by the end of the year. Curve Dental plans to make online forms available to all customers at no additional charge for a limited time only.
